Result of Varus Osteotomy in Legg-Calve- Perthes' Disease


Abstract
Legg-Calve- Perthes' disease is a self-limited disease of the hip, but coxa vara deformity, greater trochanteric overgrowth can develop, and it causes leg length discrepancy, abductor weakness, and limping etc.
Varus derotational osteotomy for the affected children is popular surgery nowadays to achieve containment. However, there may be disadvantages of the method: failure of surgical varus to resolve, limb shortening and abductor weakness. The aim of
this
paper is to evaluate the results of varus osteotomy and to evaluate the change of neck-shaft angle. The authors have analyzed 53 patients who underwent a femoral osteotomy with or without derotation and report on 35 patients who had at least 2
years
follow-up.
@ES The results were as follows.
@EN 1. The materials consisted of 25 boys and 10 girls; According to Catterall classification, 5 cases were belong to group II, 19 cases to group III and the rest 11 cases to group IV. The radiologic results were as follows:
good 18 cases(52%), fair 12 acses(34%), and poor 5 cases(15%). The clinical results were good 27 cases(77%), fair 6 cases(17%), and poor 6 cases(5%). The difference between clinical and radiological results is thought to require long term
follow-up.
2. It was average 20 degrees of surgical varus that femoral head was contained in the acetabulum. The average preoperative neck-shaft angle was 135.5¡Æand post-operative one was 115.5¡Æ. At the last follow-up study, 7.5¡Æincrement took place and
the
incremnet of the neck-shaft angle was poor when it was below 115 degrees. The result was thought to mean that overavarization should be avoided.
3. Femoral osteotomy in LCPD is thought to be one of the good methods of treatment in that is relatively simple and encourages social adaptation without application of brace.
